remember sending one in ages ago (but have yet to order any of the "returned" products).

I can look it up to see if you
have one on file if you think their should be one.
I think if your printer is not working it would be faster and
more economical for you to use the library and print the form and then mail it in. Thanks
As long as we have a copy on file it should be fine. We do occasionally miss place one and have to have the person resend a new one.
